How to Refer Someone to The RCT

If you have trouble with the link simply go to the Otago Community Hospice web page www.otagohospice.co.nz and click on 'patient services', then 'residential care information'.

Key information to include:

Name, Date of Birth, NHI, the name of your facility,

Diagnosis - if known, and the really important part- the concerns and assistance required.

Once we receive a new referral, we do some background work to learn more about the person. We may make contact to ask a few questions and advise when we will visit.
