Important update
In response to some upcoming changes in the Privacy Laws the RCT will be adjusting the form used to refer residents to the Otago Community Hospice Residential Care Team. This will include:
EXTRA information referrers will need to add:
Acknowledging the resident and/or their family/whānau (especially if EPOA has been invoked) CONSENT to being referred to the Hospice team (including that Hospice can access the residents health information to guide care)
Confirming with the family/whānau that their contact details can be passed onto the Hospice team
Acknowledging that you or your manager has notified the residents GP that you are referring the resident to the Hospice
Once the new form is active referral will not be accepted without the above additions. This is because we need residents and whānau documented consent for us to be involved in their care and access their health records.

Note: People already under Otago Community Hospice care who move to residential care from their home in the community will NOT need a new referral.
