An opportunity to address problems

With the increasing complexity of residents’ care needs, a team approach to care and treatment is always beneficial, but care facilities may feel isolated or unsure of how best to discuss individual cases, particularly when complex. Resident case review meetings can provide an opportunity to address problems that are of most concern to the resident or those providing the care.

Multidisciplinary team (MDT) meetings have been promoted as the optimal forum for complex resident case discussions. There is compelling evidence that this multidisciplinary approach helps to enhance a person’s quality of life by addressing the problems that are of most concern to them.

The meeting is designed to be a supportive, welcoming forum to troubleshoot the residents’ problems or concerns. It is not intended to criticise or critique care provision but to be more of a ‘’brain storming’ session where solutions that might be helpful or worth adding into the person’s care plan can be discussed.

Multidisciplinary team (MDT) meetings often include multidisciplinary team members, but nurses and health care assistants, can get together and discuss complex or challenging cases using the MDT model. Some facilities hold resident case reviews or MDT meetings on their own, but if you would like help setting up a resident review meeting then please make contact with the RCT nurses. We would be more than happy to be involved and provide support.

If you are uncertain how to present a resident or outline your concerns, the below template might assist you with this (see separate document).
